Medical Records Requests
Requesting Records Via Fax/Mail
To start your request, simply download, print, complete, and sign the Records Request Form.
Please be sure to sign the form, unsigned requests cannot be processed. Please include a copy of your valid ID card or driver license.
Fax the form to: (855) 698-4171 - or - Mail the form to: Attention: Medical Records, 500 W. Hospital Road, French Camp, CA 95231
Your request will be processed and fulfilled within 15 business days from the day it is received. Please allow reasonable time to process your request. We will mail your medical records to the address specified on the authorization form. An invoice may accompany medical records requests.
